NeilBrown 80ca3a44f5 [PATCH] md: unify usage of symbolic names for perms ...

Some places we use number (0660) someplaces names (S_IRUGO).  Change all
numbers to be names, and change 0655 to be what it should be.

Also make some formatting more consistent.
